NOTICE
JJP Family Practice, will close its Rural Health Clinic effective November 1, 2025.  After that date, JJP will continue to operate as a medical practice and will still provide professional and related ancillary services for its patients.
If you have any questions, call the clinic at 361-358-9912.  Medicaid members who need help finding a new RHC provider may call 2-1-1 Texas (or 877-541-7905).
This notice is provided consistent with CMS RHC requirements and HHSC guidance.
